Autumn Leaves is a family owned and operated residential environment dedicated to providing the highest quality care for individuals living with memory impairment.

At Autumn Leaves we want our residents to feel comfortable and secure. We believe that creating a home-like environment allows them to maintain their own identity and preserve their dignity, while staying active. At Autumn Leaves we believe in an age in place philosophy. We design our homes with the needs of our residents and their families in mind- from a private dining room for family occasions and protected outdoor walking paths…to nutrition and medication monitoring, organized activities and much more. Each community is home to between 42 and 44 residents. The environment that we create in each of our homes is informed by the most current research in design specifically for dementia.

We try to provide them just the right amount of assistance so that residents can live as independently as possible. And we give them the space and freedom to pursue their hobbies in a safe, supportive, and family-like environment.

We understand that deciding to move a loved one into an assisted living environment can be a difficult process. We want you to know that when you select Autumn Leaves you are gaining an experienced care giving partner to provide for the well-being and safety of your loved one.

Amenities and Services

  • Staff trained in dementia on site 24 hours a day.
  • Licensed Registered Nurse on site 40 house a week and on call 24 hours a day.
  • Administration, storage, and reordering of call medications.
  • Health monitoring (vital signs, lab tests, therapy needs, sugar levels).
  • Assistance with daily living- from bathing, dressing, and eating to toileting, hygiene and mobility issues.
  • Three nutritious meals are prepared daily with attention to each individual’s dietary needs.

  • Scheduled activities and exercise program 7 days a week (including family incorporated activities, gardening and field trips, religious services, music and pet therapy, and much more)
